First year membership. It has a small insurance element, a budo pass and allows a member to grade while the membership is valid
 
A membership form: This form needs to be completed for each new member. Under 18s need a parent or guardian to sign the document. Ages 13 to 17 need to sign the GDPR permission. The form is sent to NAKMAS to register the membership.
 
Health Questionnaire: The health form needs to be completed prior to the first training session. A parent or guardian needs to sign the form for under 18s The form is retained locally at the club
 
Under 18s need a parent or guardian to complete the Photography And Video consent form to advise if images of the student can be used by the club
